What is "going the extra mile" when looking for a new job?

I've been looking for a job for a while now and I've applied to numerous companies on multiple job portals. Only 1% to 2% of companies respond to my applications, and even they don't go beyond the initial introduction call, at the end of which I'm told "we'll be in touch" but they never call back. I've tried referrals too but without any luck. I have 4 years of experience at KPMG (the only job I've ever had) as a consultant and I have an MBA from IIM Udaipur. I think my CV isn't terrible either. I've had it reviewed from friends, family, ex-colleagues, and seniors from my college, listened to their advice, made the changes and applied everywhere, but there's no change. Help me. I would appreciate any and every advice y'all have to offer. Whether it's the bare minimum that I've been blind to all along or a couple extra miles, I wanna shoot a real shot.
